DECLARATION

1. I know that Registration Fee and Admission Fee are Non-Refundable & I fully understand that Registration is not binding for admission. Admission would be given only when admission reporting being made in stipulated time.
2. I have made careful note of various details regarding the payment of School Fee. I have made satisfactory arrangements for remittance for school fee within due dates without waiting for remainder from the school. I'll pay the all school fee regularly as per fee schedule.
3. I hereby certify that the date of birth and spelling of name of my child/ward given in the form are correct to the best of my knowledge and I shall not make any request to change.
4. I hereby certify that I do not claim the Caution Money paid by me before the completation of School Session.
5. I understand that rendering false or misleading information or withholding correct information may disqualify the child from admission/education at the school.
6. I certify that my ward is not suffering from any mental disability/disorder (e.g. Depression. Anxiety, Schizophrenia, Post Traumatic Stress.)
7. Having read carefully the rules, regulation and procedures as laid down in the school prospectus and being desirous of having my child/ward educated at Dedicated Public School, I hereby agree to abide by them in all respects. I understand that the decision of the Management of the School shall be final and binding on me.
8. I hereby certify that my ward and I shall follow all the rules, regulations and procedures laid down by from time to time.
9. I certify that I am the bonafide guardian of the child.
10. I hereby put my signature to confirm the above declarations.

UNDERTAKING

1. The under mentioned documents to be submitted with this application form:
   • (a) The Birth Certificate in original (Nursery, Jr. Kg and Sr. Kg) along with a photocopy.
   • (b) Transfer Certificate from previous school (class 1 onwards) duly signed by Principal and countersigned by Distt. Education Officer (DEO) of the concerned district.
   • (c) Progress Card issued by the school last attended (if any).
   • (d) Aadhar Card (Xerox copy).
   • (e) Medical Fitness Certificate (Mental Fitness) including Blood Group.
   • (f) Two copy of photograph size 3 cm x 2.5 cm.

   Note: In case of any of the above certificate is not furnished along with the application form, the same will be submitted within one month from the date of submission otherwise admission will be rendered invalid/cancelled.

2. On acceptance of this application, I agree to:
   • (a) Abide by all the rules and regulations regarding the student's attendance and discipline laid down the school.
   • (b) Pay the Annual Dev./Misc./Tuition Fee for whole course, Exam fees and allied charges as indicated in the fee schedule for the current academic session.
   • (c) Pay complete current session fee in case of midterm withdrawal of my ward.
   • (d) Fee Detail for Academic Year 2026 - 27:
     Fee + Annual Tuition Fee (Nur to Sr.KG) + Exam Fee (T1 + T2): 2,400 + 8,400 + 900 = 11,700/- Fee + Annual Tuition Fee (I to V) + Exam Fee (T1 + T2): 2,400 + 9,000 + 900 = 12,300/- Fee + Annual Tuition Fee (VI to VIII) + Exam Fee (T1 + T2): 2,400 + 10,200 + 900 = 13,500/- Fee + Annual Tuition Fee (IX - X) + Exam Fee (T1 + T2): 2,400 + 11,400 + 900 = 14,700/- Fee + Annual Tuition Fee (XI) + Exam Fee (T1 + T2): 2,400 + 14,400 + 1400 = 18,200/- Fee + Annual Tuition Fee (XII) + Exam Fee (T1 + T2): 2,400 + 14,400 + 1400 = 18,200/-

     *Fee Structure are subject to change.

     *You may pay fee on quarterly installment (One time for next three months).

     *Fee for class XI & XII will be charged annually (Lump sum for one year).

     *Board Registration Fee and Exam. Fee will be extra charged for Class IX, X, XI & XII.

   • (e) For refunding the Caution money, application along the deposit receipt must be submitted at the last month (March) of the session. Otherwise it will not be considered for refund.

Dedicated Public School, Dina Vihar, Bansa, Karwandiya नियम एवं शर्तें (अप्रैल 2023 से लागू)

1. विद्यालय में एक बार नामांकन करा लेने के बाद किसी भी परिस्थिति में कोई भी जमा राशि वापस नहीं होगी और ना हीं किसी तरह की क्लास/सत्र में परिवर्तन (Adjustment)/ किसी अन्य छात्र के लिए राशि हस्तांतरित किए जाएँगे।
2. स्कूल में बच्चों का नामांकन कम से कम एक सत्र (वर्तमान अप्रैल से अगले मार्च तक) ही होती है। इस बीच स्कूल छोड़ने की अनुमति नहीं है। अगर आप नियमित फी जमा कराने में आर्थिक रुप से (Economically) सक्षम हैं तभी बच्चे का नामांकन कराएँ।
3. गाड़ी से आने-जाने की सुविधा हेतु नामांकन के समय हीं आवेदन देना होगा। आवेदन स्वीकृत होने पर ₹ 1000/- की Security Money जमा करनी होगी। गाड़ी की सुविधा भी कम से कम एक सत्र (वर्तमान अप्रैल से अगले मार्च तक) होती है। इस बीच गाड़ी छोड़ने की अनुमति नहीं है। सत्र पूरा हो जाने पर अगर विद्यार्थी आगामी सत्र से गाड़ी से नहीं आना चाहता है तो वापसी हेतु Security Money का मूल रसीद (Original Slip) आवेदन के साथ जमा करने होंगे। भली-भांति जाँच करने के बाद प्रबंधन अंतिम फैसला लेगा।
4. Annual & Misc Charges (₹ 2400/-) प्रत्येक सत्र के अगस्त महीने की 10 तारीख तक जमा करना होगा।
5. विद्यालय पूरे सत्र के लिए शुल्क निर्धारित करता है जिसे आप बारह महीने अथवा छः महीने या तीन महीने के आसान किस्तों में अग्रिम (Advance) जमा कर सकते है या विशेष परिस्थिति में शिक्षण शुल्क वर्तमान महीने (Current Month) के पहली तारीख से 10वीं तारीख तक जमा करा सकते हैं।
6. मई एवं जून तथा फरवरी एवं मार्च का फी एक साथ जमा होता है।
7. इस बात का ध्यान रहे कि किसी भी तरह के शिक्षण एवं अन्य शुल्क वर्तमान (Current Month) महीने के 10 तारीख तक विद्यालय के कार्यावधि में जमा होगा। वर्तमान महीने की 11 तारीख से 10 रुपये प्रति दिन बिलम्ब शुल्क के साथ हीं छात्र/छात्रा का फीस जमा होगा।
8. अगर बकाया राशि अगले महीने की 15 तारीख तक तक भी जमा नहीं होता है तो विद्यार्थी का नाम रजिस्टर से कट जाएगा और बच्चे को स्कूल आने से रोक दिया जायेगा। नाम कट जाने के बाद आगामी महीने के 15 तारीख तक सभी बकाया राशि के साथ ₹ 1000 (एक हजार रुपये) Re-admission charge के साथ आप पुनः नामांकन करा सकते हैं अन्यथा विद्यार्थी का नामांकन हमेशा के लिए रद्द हो जाएगा।
9. विद्यार्थियों को महीने में समान्यतः अधिकतम तीन छुट्टियों के आवेदन स्वीकार किये जाते हैं। तीन से ज्यादा छुट्टियों के लिए अभिभावक को कारण (जरूरी दस्तावेजों) मेडिकल सर्टिफिकेट सहित स्वयं कार्यालय से स्वीकृत कराने होंगे। बिना आवेदन के छुट्टी पर तीन दिनों से अधिक की छुट्टी के लिए ₹ 10 प्रतिदिन का Absent Fine देय होंगें।
10. किसी भी परीक्षा में शामिल होने से ठीक पहले उस महीने तक के सारे बकाये चुकाने होंगे, अन्यथा विद्यार्थी परीक्षा से वंचित हो सकते हैं।
11. विद्यालय द्वारा निर्धारित नियमों/अनुशासनों का उल्लंघन छात्र/छात्रा/अभिभावक द्वारा किये जाने पर विद्यालय प्रबंधन/ प्रशासन/ समिति द्वारा उन्हें विद्यालय से निष्कासित एवं/अथवा अन्य कदम (आर्थिक/कानूनी) दण्ड/कारवाई किए जा सकते हैं।
12. विद्यालय के पास सभी अधिकार सुरक्षित हैं जिसे समयानुसार परिवर्तित भी किए जा सकते हैं।
13. मैं वर्ग ....... के अभिभावक/ पिता/माता ने उपर्युक्त सभी नियम व शर्तों तथा शुल्क संबंधी सभी जानकारियों को अच्छी तरह से पढ़ एवं समझ लिया है एवं इन सभी को अमल करने के लिए मैं सहमत हूँ।
